Nursing questions

Growth is essential for a health care manager

The detail that decides the whole proposal is the word licensed. Physical therapists make this a billable clinical service rather than an amenity, and every number follows from that.

01

Licensed staffing changes what this proposal is

One word in the scenario decides the shape of the whole document, and it is licensed. A wellness room staffed by fitness instructors is an amenity: it costs money, it may help occupancy, and it generates no revenue. A studio staffed by licensed physical therapists is something else entirely, because licensed therapy delivered under a plan of care is a billable service with defined payer rules behind it. That distinction determines your revenue assumptions, your regulatory section, your staffing costs and your risk analysis, so establish it in the executive summary rather than discovering it halfway through the financials. Note also that therapy for assisted living residents is generally billed as outpatient rather than under a facility benefit, which affects both the rate and who is eligible. Establish that in the executive summary and the financial section has something firm to model. Everything downstream, from the revenue line to the regulatory section, inherits that one decision.

The financial and risk analysis is where these proposals are won or lost, and the brief is unusually specific about it, asking for financing, expected return and how risk is calculated. Take the last clause literally: state your method rather than asserting that risk is moderate. Sensitivity analysis is the accessible approach, running the return under pessimistic, expected and optimistic assumptions on the two variables that actually move it, which here are the proportion of residents who use the service and the average reimbursement per visit. Break-even occupancy and payback period both belong here too. On the market assessment, use the resident population you already have rather than a general demographic argument, since an assisted living centre knows its census, its acuity and its fall rate, and a proposal grounded in those figures beat national ageing trends. Recommend clearly at the end, because the brief asks for a recommendation and a balanced summary is not one.

Likely learning objectives

  • Recognise how licensure converts an amenity into a billable service.
  • Ground a market assessment in the organisation's own population.
  • State a risk calculation method rather than a risk adjective.
  • Deliver a clear recommendation rather than a balanced summary.

In the field of health care, one thing is for certain: innovation will continue to grow exponentially. “..Most of the innovations that healthcare leaders say they are excited about have less to do with medical advances and more to do with improving how health care is accessed, provided, and paid for in this country” (Phillips, 2015). Understanding how to support and move forward with this growth is essential for a health care manager. Research and prepare a proposal to address the following health care topic/issue: An assisted living center wants to add a fitness studio, staffed by licensed Physical Therapists. You will need to use a document template for this assignment. Templates may be found by searching Microsoft Word. A few examples include the “Business Plan” and “Project Scope Report” templates. You will need to edit the template headers to be applicable to this assignment, but retain the overall design elements to create a clean, professional-looking document. Use the following outline within your template. Bolded items should be used as section headings. Additional subcategories may be included: Executive Summary What is being proposed? Proposal Description Supporting documentation: basic financials (ex: purchase price, lease or buy options, average reimbursement rates, etc.) Market Assessment Utilization of the product or service Financial & Risk Analysis Provide a clear picture of revenues, expenses, profits, etc. How will the project be financed? What is the expected ROI? What is the financial risk? (include how risk is calculated) Recommendation Is this a viable option? How should the entity proceed? On a separate page, include a minimum of three references, in APA format.

You must proofread your paper. But do not strictly rely on your computer’s spell-checker and grammar-checker; failure to do so indicates a lack of effort on your part and you can expect your grade to suffer accordingly. Papers with numerous misspelled words and grammatical mistakes will be penalized. Read over your paper – in silence and then aloud – before handing it in and make corrections as necessary. Often it is advantageous to have a friend proofread your paper for obvious errors. Handwritten corrections are preferable to uncorrected mistakes. Use a standard 10 to 12 point (10 to 12 characters per inch) typeface. Smaller or compressed type and papers with small margins or single-spacing are hard to read. It is better to let your essay run over the recommended number of pages than to try to compress it into fewer pages. Likewise, large type, large margins, large indentations, triple-spacing, increased leading (space between lines), increased kerning (space between letters), and any other such attempts at “padding” to increase the length of a paper are unacceptable, wasteful of trees, and will not fool your professor. The paper must be neatly formatted, double-spaced with a one-inch margin on the top, bottom, and sides of each page. When submitting hard copy, be sure to use white paper and print out using dark ink. If it is hard to read your essay, it will also be hard to follow your argument.

02

What the proposal template must contain

  1. 01A proposal built on a business plan or project scope template with edited headers.
  2. 02An executive summary stating what is being proposed.
  3. 03A proposal description with basic financials including purchase, lease or buy options and reimbursement rates.
  4. 04A market assessment covering utilization of the service.
  5. 05A financial and risk analysis covering revenues, expenses, profits, financing, expected ROI and how risk is calculated.
  6. 06A recommendation on viability and how to proceed, with a minimum of three APA references.
03

Following the prescribed section headings

01

Executive summary

State the proposal and its financial headline in a page.

What the assessor is likely looking for

A summary an executive could act on alone.

02

Proposal description and basic financials

Set out the build or lease decision, equipment and staffing costs.

What the assessor is likely looking for

Lease and buy compared rather than one assumed.

03

The licensing consequence

Explain what licensed therapy staffing requires and permits.

What the assessor is likely looking for

Regulatory and billing consequences identified.

04

Market assessment

Estimate utilisation from the centre's own resident population and acuity.

What the assessor is likely looking for

Local data rather than national trend.

05

Financial and risk analysis

Model revenue, cost, financing and return, with a sensitivity analysis.

What the assessor is likely looking for

A stated method for calculating risk.

06

Recommendation

State whether to proceed and on what conditions.

What the assessor is likely looking for

A decision with conditions attached.

04

Where reimbursement and utilisation data sits

Recommended databases

  • CMS
  • KFF
  • PubMed Central
  • University Library

Search sequence

  1. 1.Check how outpatient therapy is reimbursed before assuming a rate.
  2. 2.Look up evidence on exercise interventions and falls in older adults for the utilisation case.
  3. 3.Find comparable capital cost figures rather than estimating them.
  4. 4.Read a sensitivity analysis example so the risk method is defensible.
05

Reference shortlist

These are authoritative starting points, not a ready-made bibliography. A qualified reviewer must confirm that each source fits the assignment and supports the claim beside which it is cited.

Review before citing

Payment programme structure, for the reimbursement environment.

06

Before you submit the proposal

Common mistakes

  • Treating the studio as an amenity and omitting the revenue side entirely.
  • Ignoring the payer rules that licensed therapy brings with it.
  • Building the market assessment on national demographics rather than the resident census.
  • Describing risk as moderate without stating a calculation.
  • Ending on a balanced summary where a recommendation was asked for.

Submission checklist

  • Have you established the billable-service framing early?
  • Are reimbursement assumptions sourced rather than assumed?
  • Is the market assessment built on the centre's own residents?
  • Does the risk section state a method?
  • Is there an unambiguous recommendation?
