The variance question is the assignment
Six of the reflection questions describe what you built; the seventh asks you to manage a problem, and it is the one that separates submissions. You have used more full-time equivalents than the budget allowed for the census achieved, which means either the census assumption was wrong, the acuity was higher than the matrix assumed, or the hours per patient day drifted. Diagnose before you remedy, because the three causes call for different responses and a reflection that jumps straight to cutting hours has not done the analysis. Name the units of service you used, which for a medical surgical unit is normally hours per patient day against midnight census, and note what that measure fails to capture, since admissions and discharges consume nursing time that the census never sees. Turnover on a thirty-bed unit consumes time that never appears in a midnight count. The remaining levers are real, and the sequence matters more than the list.
The constraint on the remedy is the phrase about complying with guidelines, and it is doing real work. Whatever you propose has to respect minimum staffing requirements where they apply, skill mix requirements, contractual obligations on scheduling and rest, and the plain safety evidence that reducing registered nurse hours is associated with worse patient outcomes. That rules out the obvious answer of simply staffing below the matrix. What remains is genuinely available: flexing to census in real time rather than staffing to peak, reducing premium and agency hours which cost several times base rate, shifting skill mix toward the assistive personnel the matrix permits, redistributing non-nursing tasks, and if the variance is structural rather than episodic, taking a case to revise the budgeted census on the actual data. Say which of those you would use and in what order, because a list without a sequence is not a plan.
