Screening is not diagnosis, and PCN 501 is testing that
The prompt hands you a two-sided list and the sides are not equally difficult. Advantages come easily: standardisation, comparability across clients and across time, reduced reliance on a counsellor's impression, a defensible record, brevity that fits a first session, and a common language with referring physicians and payers. Write them, but write them attached to instruments you can name, because a post that says instruments are reliable and valid without naming one is indistinguishable from every other post in the forum. The AUDIT, the DAST, the CAGE, the ASSIST and the CRAFFT for adolescents are the obvious candidates, and each has a published cut-off and a known population it was normed on. Naming the norming population is what turns a generic advantage into a specific one, because it is also the hinge of the disadvantage half. The CAGE, for instance, was developed on hospital inpatients and asks about lifetime experience, which is why it performs poorly as a measure of current severity.
The disadvantages are where the marks are, and there is one distinction the assessor is almost certainly listening for. A screening instrument tells you whether to look further. It does not diagnose. Diagnosis under the DSM requires the clinical criteria applied by a qualified clinician, and a positive AUDIT is a reason to conduct that interview rather than a substitute for it. Around that, build the rest: cut-offs derived on populations unlike your client, self-report distorted by shame, minimisation or a mandated referral, cultural and language mismatch, literacy demands, a false sense of precision from a number, and the risk that a score becomes the client's label in the chart. Close on the practical resolution, which is that instruments belong inside a clinical interview rather than in place of one, and say who is qualified to make the diagnosis they point toward. In most agencies that is a licensed clinician and not the intake counsellor who administered the screen.
